Expectations for Your First Visit!
Our office will give you a call roughly 48 hours before your scheduled appointment to confirm the date and time and ensure you are prepared for your first Coastal Foot and Ankle consultation.
All patients at Coastal Foot and Ankle are required to have updated patient intake forms on file with us. For new patients, please arrive 30 minutes early to the office to fill out these forms. If you care to fill the forms out prior to your appointment, a PDF link can be found at the bottom of this page stating "Click Here to Download Patient Intake Forms". If you have your paperwork completed prior to your arrival, please arrive 15 minutes early for your visit.

Our Middletown Office is located at 850 Aquidneck Avenue in Middletown, RI at the Middletown Commons Plaza. Our office is located at the rear of the plaza and can be reached by driving down the left side of the plaza building and taking a right. Continue forward and our office is the first business you will see on your right. There is a green awning above our door that says "Coastal Foot and Ankle" and a wooden bench to the right of our door. There is handicap-accessible parking located in front of our door. Appointment in the Fall River Office?
Our Fall River Office is located at 1822 North Main Street in Fall River, MA. We are located in the same building as the Fall River branch of The Massachusetts Registry of Motor Vehicles. After parking, there is a large set of glass doors that say "1822" above them. Our office is right after you enter those doors. Our office is located in Suite 105 on the first floor. Any questions?
